Your team updates a procedure, yet a colleague gets an answer describing the previous workflow. The cause may be an old file, access rules or an answer that nobody checked against its source. Rephrasing the prompt alone cannot solve a document versioning problem.
Make one source authoritative
Start with the document, not the chat. Name its owner, effective date, scope and the location of the approved version. Mark the old version as archived. If two conflicting procedures are both presented as current, no prompt can guarantee the right choice.
Upload only material your organization permits. Remove unnecessary personal data and secrets, and check who can access the document in the workspace.
Test the answer with control questions
After publishing a revision, ask two or three questions whose answers changed. Request the exact source passage and document date. For example: “What approvals are now required for a return? Cite the section and version used.” Open the cited passage and compare it with the current approved document.
If the answer points to an old section, investigate before simply rewriting it. Has processing of the new version completed? Is an older version still treated as active? Do the user's permissions match the intended document scope? If the answer offers no source, it does not confirm that the new procedure is being used.
Record a human decision
A small review log needs four fields: test question, expected section in the new guide, actual source cited and the document owner's decision. If a revised procedure conflicts with policy or calls for legal judgment, route it to the appropriate professional. AI can find and explain a source; it cannot approve a company rule.
